Missed Visits. (i) Provider must develop a back-up plan for each Covered Person to be implemented during missed visits, or when otherwise necessary. For purposes of this section, “missed visit,” refers to a period of one or more hours that a staff member of Provider does not furnish the home health service that a Covered Person is authorized to receive and which has been implemented. A missed visit may be due to exigent circumstances beyond any party’s control. It may also be due to a fault of Provider, the staff member, Subcontractor or Health Plan. It may also be due to a fault of the Covered Person. For example, the Covered Person refuses to allow the staff member to enter the home or to remain there after beginning work; the staff member suspects or witnesses unlawful activity in the home; or, the environment in the Covered Person’s home is such that the staff member fears for their personal safety.

Missed Visits. Well visits not cancelled 24 hours prior to the scheduled appointment may be subject to a $100.00 charge. Xxxx visits not cancelled at least two hours prior to the appointment may be subject to a $100.00 charge. New Well Patient Visits not cancelled 24 hours prior to the scheduled appointment may be subject to a $200.00 charge. Please be aware that we value our patients and their families’ time. Please come on time to your appointment, If you are more than 15 minutes late arriving , patients may not be able to be seen by the physician and therefore, will be subject to the missed visit fee.
Missed Visits. If a subject misses a scheduled visit, reschedule the subject within the same visit period. Show diligence in trying to schedule the subject for all visits, and document all attempts to contact the subject in the subject’s chart. In documentation, include dates, times, method of contact, etc. If a subject is unable to return for the final study visit, complete the Exit Case Report Form with the appropriate reason for discontinuation. If attempts to contact the subject are unsuccessful, document the date the subject is considered lost to follow-up. Complete the subject’s Exit Case Report Form after the last window (Visit 4A) closes, indicating the subject is lost to follow-up.
Missed Visits. Missed progress appointments must be made up within 2 weeks of missing. If 24 hours is not given to reschedule an appointment, the client forfeits that appointment. In the case of an emergency situation, it is the discretion of the Practitioner or Doctor to reschedule this appointment.

  • Ongoing Trunk Forecast Requirements Where the Parties have already established interconnection in a LATA, Onvoy shall provide a new or revised traffic forecast that complies with the Frontier Interconnection Trunking Forecast Guide when Xxxxx develops plans or becomes aware of information that will materially affect the Parties’ interconnection in that LATA. Instances that require a new or revised forecast include, but are not limited to: (a) Onvoy plans to deploy a new switch; (b) Onvoy plans to implement a new POI or network architecture; (c) Onvoy plans to rearrange its network; (d) Onvoy plans to convert a One-Way Interconnection Trunk group to a Two-Way Interconnection Trunk group; (e) Onvoy plans to convert a Two-Way Interconnection Trunk group to a One-Way Interconnection Trunk group; or (f) Onvoy expects a significant change in interconnection traffic volume. In addition, upon request by either Party, the Parties shall meet to: (i) review traffic and usage data on End Office and Tandem Interconnection Trunk groups and (ii) determine whether the Parties should establish new Interconnection Trunk groups, augment existing Interconnection Trunk groups, or disconnect existing Interconnection Trunks.

  • Direct Trunked Transport 7.3.2.1 Either Party may elect to purchase direct trunked transport from the other Party.
