Other Qualified Health Care Professional definition

Other Qualified Health Care Professional. (OQHP) means the following professionals (please note this list is not all inclusive): nurse practitioner (NP), certified registered nurse anesthetist (CRNA), certified registered nurse (CRN), clinical nurse specialist (CNS), and physician assistant (PA).

  • Modifier 25 is defined as: Significant, Separately Identifiable Evaluation and Management Service by the Same Physician or Other Qualified Health Care Professional on the Same Day of the Procedure or Other Service.

  • The following modifiers will be recognized for reimbursement by the fee schedule for Evaluation and Management (E/M) codes: 24 Unrelated Evaluation and Management Service by the Same Physician or Other Qualified Health Care Professional during a Postoperative Period: The physician or other qualified health care professional may need to indicate that an E/M service was performed during a postoperative period for a reason(s) unrelated to the original procedure.

  • To report a separate and distinct E/M service with a non-E/M service performed on the same date, see modifier 25.76 Repeat Procedure or Service by Same Physician or Other Qualified Health Care Professional: It may be necessary to indicate that a procedure or service was repeated by the same physician or other qualified health care professional subsequent to the original procedure or service.

  • Note: This modifier should not be appended to an E/M service.77 Repeat Procedure by Another Physician or Other Qualified Health Care Professional: It may be necessary to indicate that a basic procedure or service was repeated by another physician or other qualified health care professional subsequent to the original procedure or service.

  • Note: This modifier should not be appended to an E/M service.24 Unrelated Evaluation and Management Service by the Same Physician or Other Qualified Health Care Professional During a Postoperative Period: The physician or other qualified health care professional may need to indicate that an E/M service was performed during a postoperative period for a reason(s) unrelated to the original procedure.

  • The control of bleeding may be separately reportable with the use ofmodifier 78 (Unplanned Return to the Operating/ Procedure Room by the Same Physician or Other Qualified Health Care Professional Following Initial Procedure for a Related Procedure During the Postoperative Period).Modifiers may be appended to HCPCS/CPT codes only if the clinical circumstances justify the use of the modifier.

  • Add an additional 20% to the value of the procedure code when billed with this modifier.24 Unrelated Evaluation and Management Service by the Same Physician or Other Qualified Health Care Professional During a Postoperative Period: The physician or other qualified health care professional may need to indicate that an evaluation and management E/M service was performed during a postoperative period for a reason(s) unrelated to the original procedure.

  • To report a separate and distinct E/M service with a non-E/M service with a non-E/M service performed on the same date, see modifier 25.76 Repeat Procedure or Service by Same Physician or Other Qualified Health Care Professional: It may be necessary to indicate that a procedure or service was repeated by the same physician or other qualified health care professional subsequent to the original procedure or service.

  • Modifier 78 [Unplanned Return to the Operating/Procedure Room by the Same Physician or Other Qualified Health Care Professional Following Initial Procedure for a Related Procedure During the Postoperative Period] is appended to 61618-61619 when the return to the operating room occurs during the post-operative global period of the prior skull base procedure.
