EXTENDED ILLNESSES. Students whose physicians certify that they are unable to attend school because of illness, accident, or pregnancy, may apply for homebound instruction. Applications are available at school or at the district office. HOMEBOUND FORMS MUST BE SUBMITTED WITHIN TEN DAYS OF THE STUDENT’S FIRST ABSENCE FROM SCHOOL.

EXTENDED ILLNESSES. Any employees who as a result of extended illness or disability has used all of his/her accumulated sick leave and who requires additional time off due to such illness, shall be allowed up to twelve (12) months unpaid leave if requested in writing. If the employee does not return from the approved unpaid leave, he/she shall be considered terminated. Any employee hired to take the place of an employee off on paid or requested and granted unpaid sick leave shall enjoy employment only for the term of the sick leave. Upon return from the leave, the employee may be requested to provide verification of the ability to perform duties of position and will return to vacancy in the group held at the commencement of leave assuming that the returning Employee is qualified to perform the duties of the position.

EXTENDED ILLNESSES. The Employee will furnish an acceptable medical certificate if the Employee utilizes sick leave for more than three (3) consecutive workdays, notwithstanding the fact that those days may be interrupted by day(s) off. Firefighters must provide an acceptable medical certificate if the Employee utilizes sick leave for more than two (2) consecutive scheduled shifts. Except for the exception specified in SECTION 8 below; each Employee shall furnish an acceptable medical certificate within three (3) days after they return to work. Sick leave will be granted for all periods of absence due to sickness or injury covered in the medical certificate.

TARDINESS: Please make sure students attend school on time each day. School begins at 7:35 a.m. and students are marked as tardy if they are not in class and seated by 7:45

  • Extended Illness Sick leave for extended illness (5 or more days) will be paid only during the time period in which a physician certifies the employee to be physically or mentally disabled, and only to the extent of the number of days accumulated.

  • Extended Illness Bank Employees who were hired prior to January 15, 1984 and who have hours remaining in their extended illness bank may access those hours in accordance with the Employer’s policy. Hours do not accrue in these extended illness banks.

  • Extended Illness Leave 1. Extended Illness Leave, hereinafter referred to as “EIL” shall be established for Administrative Support Group, XXXX, Clerical, and Educational Assistant personnel who are absent due to their own personal illness or injury following the exhaustion of their accumulated illness, vacation and annual allotment of short term leave. Eligibility for Extended Illness Leave includes completion of employee’s probationary period.

  • Extended Leaves Extended leaves of absence of at least three (3) but not more than five (5) years may be granted in accordance with Minnesota Statutes § 136F.43. Notwithstanding any other section of this Agreement, retention and accrual of all rights and benefits for faculty on extended leave shall be governed by Minnesota Statutes § 136F.43. Faculty members shall be returned to the unit in accordance with Article 29, Section B, Subd. 1, and shall be returned to the salary schedule at the step at which they were placed when they left the unit, with accommodation for general increases in the indicated step. If the faculty member’s service to the university in the year the faculty member left the unit qualified for an increase in the succeeding year, then the faculty member shall be entitled to that increase upon return to the unit. Seniority shall not accrue during the years faculty members are on extended leave.

  • Extended Sick Leave When sick leave extends for more than 25 consecutive working days, the appointing authority shall initiate the following procedure:

  • Extended Hours Depending upon local service capabilities, available extended hour plans include 5-, 6-, and 7-day, 8-, 16-, and 24-hour options at additional cost. Alaris will use commercially reasonable efforts to provide Service outside of Agreement Hours as shown below. Any such service performed will be billed at prevailing Alaris Per Call Overtime rates.

  • SHORT-TERM ILLNESS AND INJURY AND LONG-TERM DISABILITY Employees shall be entitled to coverage for short term illness and injury and long term disability in accordance with agreed upon regulations which will be subject to review and revision during the period of this Agreement by negotiations between the Parties and included as Appendix A to this Agreement.

  • Sick Leave Days Payable at 100% Wages Permanent Employees Subject to paragraphs d), e) and f) below, Employees will be allocated eleven (11) sick days payable at one hundred percent (100%) of wages on the first day of each fiscal year, or the first day of employment.

  • Extended Absences In the event Tenant will be away from the Premises for more than 7 consecutive days, Tenant agrees to notify Landlord in writing of such absence. During such absence, Landlord may enter the premises at times reasonable necessary to maintain the property and inspect for damages and needed repairs.

  • SHIFT AND WEEKEND PREMIUMS 39.1 An employee working on shifts shall be paid a shift premium of two dollars ($2.00) per hour for all hours worked, including overtime hours, between 16:00 and 08:00. The shift premium will not be paid for hours worked between 08:00 and 16:00.
