Annual Physical. The Executive may, if the Executive so elects, within the twelve (12) months following the Date of Termination, receive an annual physical at the Company’s expense consistent with the physical provided under, and subject to the requirements of, the Company’s annual physical program as in effect immediately prior to the Date of Termination.
Annual Physical. The Company shall provide, at its cost, a medical examination for the Executive on an annual basis by a licensed physician in the Scottsdale or Phoenix, Arizona area selected by the Executive; provided that the expense for such annual physical shall not exceed $1,500 per year or such additional amount as may be subsequently approved by the Board or a committee thereof.

Annual Physical. Members of the bargaining unit and their covered dependents shall be provided a free annual preventive examination provided such is performed by a network physician whether billed as medical or routine. A medical diagnosis will not prevent full payment.
Annual Physical. Executive shall be entitled to be reimbursed by the Company for the full cost of an annual physical examination by a physician (1) selected by the Company or (2) selected by Executive and approved by the Company.

If the Executive desires an annual physical examination, the Company shall provide, at its cost, a medical examination for the Executive on an annual basis by a licensed physician in the New York, New York metropolitan area selected by the Executive. The results of the examination and any medical information or records regarding the examination will be provided by the physician to the executive, and not to the Company.

You are encouraged to take an annual physical examination from a physician at the Company's expense and to certify in writing to the Company's designee each year (1) that you have had the examination and (2) the nature and extent of any medical impairments that prevent you from currently performing the essential functions of your position.
Annual Physical. The Company will pay the expenses associated with an annual physical examination for the Employee.
Annual Physical. The Executive agrees to have an annual physical examination performed by a physician of his choice during each year of this Agreement. The Company shall reimburse Executive for the costs of his annual physical examination.
Annual Physical. If the initial and annual physical examinations are not provided for by the State, the employee may have the examination done by a physician of his/her own choice without loss of compensation. However, if the initial and annual physical examinations are made available by the State and the employee prefers his/her own personal physician, then the time spent for this purpose shall be charged to sick leave and the cost of the examination shall be at the expense of the employee. An employee must have prior approval, for operational reasons, to leave the work site to take a physical examination.
