Through a sub-contract arrangement with the Winnipeg Regional Health Authority (WRHA), Manitoba will provide a Toolkit and supporting documentation and training to enable the RHA to facilitate the engagement of Clinics in the Interprofessional Team Demonstration Initiative and to support them in integrating the Interprofessional Team Member into the Clinic’s operations.
The Local Public Health Authority (LPHA) must partner with their regional Coordinated Care Organizations (CCO) and local community-based organizations (CBOs) to align and delineate organizational roles and responsibilities to improve health outcomes, while leveraging existing community-wide health improvement initiatives.
LPHA must complete an “Oregon Health Authority Public Health Division Expenditure and Revenue Report” located in Exhibit C of this Agreement.
Any funding required by the Society to augment the difference between the Tenant Rent Contribution less Tenant Shelter Contribution, and the cost of providing support services to the Tenant, is the responsibility of the Society and/or the Health Authority pursuant to the Health Agreement.
