Services and Activities. In the case of accident or injury to the named participant, and if an emergency contact person cannot be reached, I grant College Racquet Club permission to obtain medical attention if necessary for which I will be financially responsible. College Racquet Club reserves the right to cancel this contract at any time, at its sole discretion, and College Racquet Club’s sole liability shall be to refund any amounts previously paid on a pro-rata basis. College Racquet Club reserves the rights to close courts for repair or alteration. College Racquet Club reserves the right to photographs or video taken at the facility to be used for publicity or advertising. If the named participants or participant’s legal guardian’s email address are provided above, College Racquet Club is authorized to contact them at such address directly.

Services and Activities. The Contractor shall provide all necessary materials, equipment, supplies, and labor resources to provide the following professional top-quality fundraising products, services, and activities. Contractor shall list the type(s) of products/services/activities to be provided below; e.g., brochure sales—pizza, cookie dough, gift items, candles; purchase for resale—holiday wreaths, etc.
Services and Activities. Transitional housing will be provided by the Service League in small, safe, single-sex shared houses. The maximum length of stay is twelve (12) months; however program participants will be assisted in implementing aftercare plans that will include preparing to live independently as soon as possible. As a sober living facility, program participants must be successful graduates of substance abuse treatment, must follow house rules, and must not use inside or outside the house during their stay. Program participants will contribute partial rent, which supports both the facility and support services. Program participants will purchase their own food and xxxx for themselves in a modern shared kitchen. Because the service provided is transitional housing, rather than alcohol and drug treatment, the program will not offer alcohol and drug treatment services on-site. However, the program will offer a variety of services off-site in which program participants must participate. Similarly, the program is not licensed as a residential care facility, therefore the Contractor will not offer any medical or nursing care. Program participants must be healthy enough to live independently. Program participants with HIV/AIDS who become too ill to live independently while in the program will be referred to acute care hospitals, nursing homes, residential care facilities, or other appropriate venues. Placement of these program participants will be discussed with their medical provider and, if appropriate, their AIDS Case Manager. In some cases, other agencies may be able to provide limited medical care and home health aides to permit program participants to remain during periods of such illness.

In the event an inmate with a mobility disability is assigned to non-ADA housing or a specific accommodation cannot be met, Classification and PMU staff will ensure that an inmate deemed to require accessible accommodations is provided with such accessible accommodations as soon as possible.” The revised and implemented Custody & Court Operations Manual (CCOM) 8000 - Inmates with Disabilities states, “Inmates with a disability shall be housed in the most integrated setting appropriate, consistent with their individual security classification, in facilities that accommodate their disabilities and in which they have equivalent access to programs, services, and activities. Services and Activities. County shall provide the following services (the “Services”): Within no more than twenty-four (24) hours of administering a dose of COVID-19 vaccine and adjuvant (if applicable), County shall record in the vaccine recipient’s record, and shall report complete and accurate vaccine administration data required in the MyTurn reporting tool and other technology platforms required by Agency and CDC (such as, for example, VaccineFinder, and CAIR2) or other appropriate electronic health record interface as permitted under subparagraph E of this Section 3. Agency acknowledges that there may be circumstances that cause delay in the submission of data, in which case County will alert Agency and complete data recordation and submission as soon as practicable; and County shall make available at its vaccination site or sites written materials that Agency prepares, prints, and delivers to County; and County shall provide additional support as may be reasonably requested by Agency to assist Agency in the successful COVID-19 vaccine administration.
