Prior Authorization Process for Select Formulary and Non. Formulary Drugs and Most Home Self- Administered Injectables Select Formulary Drugs, as well as most Home Self- Administered injectables may require prior authorization for Medical Necessity. Select Non-Formulary Drugs may require prior authorization for Medical Necessity, and to determine if lower cost alternatives are available and just as effective. Compound drugs are covered only if the requirements listed under the Exclusions in the Outpatient Prescription Drug Benefits section are met. Your Physician may request prior authorization by sub- mitting supporting information to Blue Shield. Once all required supporting information is received, prior author- ization approval or denial, based upon Medical Necessi- ty, is provided within two business days.
