Optional Life Insurance. The State shall make available optional term-life insurance to employees. The cost will be paid by the employee on a payroll deduction basis. The available coverage will be at least two (2) times the employee’s salary. No evidence of insurability will be required if an adequate number of employees participate. The State will explore smoker/non-smoker rates and spousal coverage.
Optional Life Insurance. Subject to the approval of the insurance carrier, optional life insurance shall be available in units of $25,000 to a maximum of $250,000 provided that the employee pays 100% of the premium costs.
Optional Life Insurance. The Employer agrees to provide coverage for purchase by employees for insurance for employees and their eligible dependant spouse in units of $10,000 to a maximum of $700,000 per covered individual provided the employee pays the full premium.
Optional Life Insurance. So long as permitted by the school district's group insurance carrier, full-time teachers may purchase additional life insurance coverage on their own lives or on the lives of their spouse and/or children. The cost of the premium for this optional life insurance shall be borne by the employee and paid by payroll deduction. The premium cost, underwriting conditions, and insurance contracts shall be determined by the school district's group life insurance carrier. Any disputes that may arise between the carrier and the employee shall not involve the school district.
Optional Life Insurance. The Employer shall offer a group optional life insurance policy for each full-time employee and the employee’s dependents, at the employee’s cost.
Optional Life Insurance. Optional term life insurance for employees shall be administered in accordance with the Benefits Trust.
Optional Life Insurance. To the extent that the State of Ohio offers an optional term life insurance plan, the same plan will be offered to employees in the bargaining unit.
Optional Life Insurance. The Hospital will make available an Optional Life Insurance plan to employees, at no cost to the Hospital.
Optional Life Insurance. Insurance eligible employees may purchase additional life insurance in $10,000 increments up to $100,000 in coverage. The amount of coverage existing employees may purchase with evidence of good health will be determined by the insurance carrier. Evidence of good health for new employees is not required for supplemental life if applied for during the first thirty (30) days of employment.
Optional Life Insurance. The Employer will make every effort, but does not guarantee, to permit any bargaining unit employee who desires to purchase at their cost additional life insurance through the County Plan.
