Notices Regarding Business Associates. Trading Partner will, prior to commencement of Business Associate’s services in connection with this Agreement, submit a complete, executed Business Associate Authorization form (Exhibit B of this Agreement) designating each Business Associate authorized to send or receive Data or Data Transmissions to or from UHA on Trading Partner’s behalf. Trading Partner will notify UHA at least fourteen (14) days prior to the date of any material change to the information contained in the Business Associate Authorization form. The Business Associate Authorization form will be effective and incorporated into this Agreement on the date it is received by UHA.

Notices Regarding Business Associates. Trading Partner shall, prior to commencement of a Business Associate’s services in connection with this Agreement, submit a complete, executed Trading Partner Business Associate Authorization Rider (“Business Associate Rider”) designating the Business Associate as an entity authorized to send or receive data or Data Transmissions to or from BCBSSC on Trading Partner’s behalf. Trading Partner shall notify BCBSSC at least fourteen (14) days prior to the date of any material change to the information contained in the Business Associate Rider. The Business Associate Rider will be effective and incorporated into this Agreement on the date it is received by BCBSSC. Trading Partner’s designation of any Business Associate for purposes of performing services in connection with this Agreement is subject to BCBSSC’s approval. BCBSSC will not unreasonably withhold approval of Trading Partner’s designated Business Associates.

PGBA, LLC will not unreasonably withhold approval of Trading Partner’s designated Business Associates.

At the request of University, Contractor agrees to provide University with a written summary of the procedures Contractor uses to safeguard and maintain the confidentiality of University Records.

  • Contact Information for Privacy and Security Officers and Reports 2.1 Business Associate shall provide, within ten (10) days of the execution of this Agreement, written notice to the Contract or Grant manager the names and contact information of both the HIPAA Privacy Officer and HIPAA Security Officer of the Business Associate. This information must be updated by Business Associate any time these contacts change.
