NEW BUSINESS UNITS OR LOCATIONS. In the event that, after the Effective Date, Gambro changes locations or sells, closes, purchases, or establishes a new business unit or location related to the furnishing of outpatient dialysis, Gambro shall enter this information into a log as soon as possible, but no later than within 15 days after the date of change of location, sale, closure, purchase, or establishment. Each log shall include the address of the new business unit or location, phone number, fax number, Medicare number, Gambro identification number and/or supplier number, the corresponding contractor’s name and address that has issued each Medicare number, and the name, address, phone number, and provider and/or supplier number (if applicable) for each business partner of the new business unit or location. NEW BUSINESS UNITS OR LOCATIONS. In the event that, after the Effective Date, Maximus changes locations or sells, closes, purchases, or establishes a new business unit or location related to the furnishing of items or services that may be reimbursed by Federal health care programs, Maximus shall notify OIG of this fact as soon as possible, but no later than within 30 days after the date of change of location, sale, closure, purchase, or establishment. This notification shall include the address of the new business unit or location, phone number, fax number, each new business unit or location shall be subject to all the requirements of this CIA.
NEW BUSINESS UNITS OR LOCATIONS. HealthSouth shall notify OIG on a quarterly basis of the purchase, sale, closure, establishment, or relocation of any HealthSouth facility furnishing items or services that may be reimbursed by Federal health care programs. This notification shall include the address of the affected facility, phone number, fax number, Medicare Provider number, provider identification number and/or supplier number, and the corresponding contractor’s name and address that has issued each Medicare number. Each new HealthSouth facility shall be subject to the requirements of this CIA.
NEW BUSINESS UNITS OR LOCATIONS. In the event that, after the Effective Date, King changes locations or sells, closes, purchases, or establishes a new business unit or other location engaged in Government Pricing and Medicaid Drug Rebate Related Functions or in the promotion, sales, or marketing of items that may be reimbursed by Federal Health Care Programs (collectively “Relevant Activities”), King shall notify OIG of this fact as soon as possible, but no later than within 30 days after the date of change of location, sale, closure, purchase, or establishment. This notification shall include the address of the new business unit or other location, phone number, fax number, Federal Health Care Program provider or supplier number (if any), and the corresponding contractor’s name and address that has issued each provider or supplier number. Each new business unit or location that is engaged in the Relevant Activities shall be subject to all the requirements of this CIA. King shall use its best efforts to implement the requirements of this CIA in new business units or other locations engaged in Relevant Activities. Notwithstanding any other provisions to the contrary, the requirements of this CIA shall not become effective for new business units or other locations until 120 days after the purchase or establishment or acquisition of such new business units or locations.

Prior to purchasing, establishing, selling, or divesting a facility, Bevexxx xxxll notify the OIG in writing of the proposed action. This notification shall include the location of the existing or new operation(s), phone number, fax number, Federal health care program provider number(s) (if any), and the corresponding payor(s) (contractor specific) that has issued each provider number. Bevexxx xxxll further notify the OIG in writing once such proposed purchase, establishment, sale, or divestiture has been completed.

This notification shall include the address of the new business unit or location, phone number, fax number, Medicare Provider number, provider identification number and/or supplier number, and the corresponding contractor’s name and address that has issued each Medicare number. Each new business unit or location shall be subject to all the requirements of this CIA.
NEW BUSINESS UNITS OR LOCATIONS. In the event that LifePoint: (1) purchases or establishes a new hospital, freestanding ambulatory surgery center, home health agency, or another line of business that provides services that are billed to Federal health care programs; or (2) sells or divests an existing hospital, freestanding ambulatory surgery center, or home health agency, LifePoint shall notify OIG of this fact within 30 days of the date of purchase, establishment, sale, or divestiture. This notification shall include the location of the operation(s), telephone number, fax number, Federal health care program provider number(s) (if any), and the corresponding payor(s) (contractor specific) that has issued each provider number. All Covered Persons at new locations shall be subject to the requirements in this CIA that apply to new Covered Persons (e.g., completing certifications and undergoing training). If LifePoint sells all of the assets related to a location, then that location shall no longer be considered part of LifePoint for the purposes of this CIA. If the location is still owned or operated in whole or in part by LifePoint or any of its subsidiaries, affiliates, or their successors, then the location shall continue to be considered part of LifePoint for the purposes of this CIA. In the event that, after the effective date of this CIA, Tender Loving Care changes locations or purchases or establishes new business units related to the furnishing of items or services that may be reimbursed by Federal health care programs, Tender Loving Care shall notify OIG of this fact as soon as possible, but no later than within 30 days of the date of change of location, purchase or establishment. This notification shall include the location of the new operation(s), phone number, fax number, Medicare provider number(s) (if any), and the corresponding contractor's name and address that has issued each Medicare provider number. All Covered Persons at such locations shall be subject to the applicable requirements in this CIA (e.g, completing certifications and undergoing training).

In the event that CHS purchases or establishes new business units after the effective date of this CCA, CHS shall notify OIG of this fact within 30 days of the date of purchase or establishment. This notification shall include the location of the new operation(s), telephone number, facsimile number, Federal health care program provider number(s) (if any), and the corresponding payor(s) (contractor specific) that has issued each provider number. All Covered Persons at such locations shall be subject to the requirements in this CCA that apply to new Covered Persons (e.g., completing affirmations and certifications and undergoing training).
