Name and Address Changes. It is your responsibility to notify the Credit Union upon a change of address or change of name. The Credit Union is only required to attempt to communicate with you only at the most recent address you have provided to the Credit Union. The Credit Union will accept notices of a change in address and any other notice from you to the Credit Union only if provided in writing to the Credit Union. If the Credit Union attempts to locate you, the Credit Union may impose a service fee as set forth on the Rate and Fee Schedule.

Name and Address Changes. When names or addresses change, the changes should be reported to the Credit Union's Credit Card Department. Address changes may be noted on the payment envelope used to remit payments, but it is helpful to notify the Credit Union, also. All name changes must be reported to the Credit Union.
Name and Address Changes. Any Account Holder or person authorized to sign on an Account is required to notify us in writing if any account holder or other person authorized to sign on an Account dies or is declared incompetent by a court.
