Life Event. The words "Life Event" when used in this agreement shall mean: 🞟 Marriage or cohabitation with a common-law spouse for a continuous one (1) year period 🞟 Birth, adoption or change in custody of a dependent child 🞟 Divorce 🞟 Legal separation or the ending of a common-law relationship 🞟 Death of a spouse or dependent child 🞟 Involuntary loss of a spouse's or dependent child's coverage under the spouse's employer's plan, or 🞟 Dependent no longer qualified as a dependent under the plan.
Life Event. The words “Life Event” when used in this Agreement shall mean: 🞟 Marriage or cohabitation with a common-law spouse for a continuous one-year period; 🞟 Birth, adoption or change in custody of a dependent child; 🞟 Divorce; 🞟 Legal separation or the ending of a common-law relationship; 🞟 Death of a spouse or dependent child; 🞟 Loss of a spouse’s or dependent child’s coverage under the spouse’s employers’ plan; 🞟 Dependent no longer qualifies as a dependent under the plan; or, 🞟 Spouse becomes eligible for benefits that are at least equivalent to the coverage provided under the City’s plans. 3.10 Member‌ The word “member” when used in this Agreement, in reference to a specific Plan contained herein, shall mean an individual who, through the individual’s employment with the City, has entered into participation in such Plan in accordance with the requirements of such Plan and has continued to participate in such Plan. 3.11 Monthly Salary‌ The words “monthly salary” when used in this Agreement shall mean: (Bi-weekly pay at regular rate of pay times 26.1) divided by 12 = monthly salary 3.12

  • Changes Due to a Life Event After the initial enrollment period and outside of any open enrollment period, an employee may elect to change health or dental coverage (including adding or canceling coverage) and any applicable employee contributions in the following situations (as long as allowed under the applicable provisions, regulations, and rules of the federal and state law in effect at the beginning of the plan year). The request to change coverage must be consistent with a change in status that qualifies as a life event, and does not include changing health or dental plans, which may only be done under the terms of Section 5A above. Any election to add coverage must be made within thirty (30) days following the event, and any election to cancel coverage must be made within sixty (60) days following the event. (An employee and a retired employee may add dependent health or dental coverage following the birth of a child or dependent grandchild, or following the adoption of a child, without regard to the thirty (30) day limit.) These life events (for both employees and retirees) are:

  • Starting Date Unless a specific (fixed) starting date is duly justified and agreed upon during the preparation of the Grant Agreement, the project will start on the first day of the month following the entry info force of the Grant Agreement (NB : entry into force = signature by the Commission). Please note that if a fixed starting date is used, you will be required to provide a detailed justification on a separate note.

  • Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion - Lower Tier Covered Transactions (a) The prospective lower tier participant certifies, by submission of this proposal, that neither it nor its principals is presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in this transaction by any federal department or agency.

  • Required Beginning Date The Participant’s entire interest will be distributed, or begin to be distributed, to the Participant no later than the Participant’s required beginning date.

  • Dependent Eligibility To be eligible to enroll as a Covered Dependent, a person must be:

  • TERM, TERMINATION & SUSPENSION The Company may terminate this Agreement with You at any time for any reason, with or without cause. The Company specifically reserves the right to terminate this Agreement if You violate any of the terms outlined herein, including, but not limited to, violating the intellectual property rights of the Company or a third party, failing to comply with applicable laws or other legal obligations, and/or publishing or distributing illegal material. If You have registered for an account with Us, You may also terminate this Agreement at any time by contacting Us and requesting termination. At the termination of this Agreement, any provisions that would be expected to survive termination by their nature shall remain in full force and effect.

  • Extension After Suspension When a suspension occurs through no fault of PURCHASER, PURCHASER may request an extension of time for performance of this contract, for a period not to exceed the period of operations that were suspended. The request for extension must be in writing and:

  • Suspension; Cancellation; Acceleration of Maturity Section 5.01. The following are specified as additional events for suspension of the right of the Borrower to make withdrawals from the Loan Account for the purposes of Section 8.01(m) of the Loan Regulations:

  • Non-Paid Status During Treatment After Positive Test The employee will be in a non-pay status during any absence for evaluation or treatment, while participating in a rehabilitation program.
