Form Contract/Duties of Contractor. The Contractor shall provide the Quality Assurance Services set forth in the respective Entity Scope of Work subject to the Form Contract attached and incorporated as Exhibit B. Any agency utilizing the contract shall complete the Scope of Work (SoW) attached and incorporated fully as Exhibit A. Once paperwork is fully executed, the Contractor shall develop a project plan for each new business opportunity added to this Contract with timelines mutually agreed to by the State and the Contractor. The executed SoW’s are deemed to be attached to and incorporated within this Contract by reference. Any inconsistency, conflict, or ambiguity between the SOW(s) and this Contract shall be resolved by giving precedence and effect to this Contract, provided, however, that the SOW may contain more restrictive or agency-specific provisions regarding confidentiality and privacy of information.

Form Contract/Duties of Contractor. The Contractor shall provide the Services or Products described in the Form Contract. In addition to the services described in the Form Contract, the Contractor agrees to provide the State the services described herein, and at no additional charge unless otherwise noted. While providing the travel services described herein and in the previously referenced Form Contract, the Contractor agrees to make its best efforts to assist Indiana Department of Administration’s Travel Office in maintaining statewide compliance with the State’s travel policies. The State will be responsible for informing the Contractor about State travel policies, processes and the preferences of the State Travel Office. The Contractor shall not assist any State travelers in booking of any flights, hotel reservation, vehicle reservations, or conferences that contradict any policies or preferences that have been shared with the Contractor by the State Travel Office. The Contractor agrees to notify the State Travel Office of any notable trends regarding policy compliance. The Contractor shall establish partnerships with hotels, airlines, and parking entities to receive discounted rates. The Contractor, upon the State’s request, shall research best rate opportunities on behalf of the State for airline fares, ground transportation, car rentals, individual hotel reservations, group hotel reservations and conference sites in compliance with the Indiana Department of Administration Travel Policy, as the same may be periodically amended. The Contractor shall make best efforts to maintain, if not expand, the existing preferred hotel program, which consists of hotels throughout the State at which State employees can reserve rooms at the State rate published in the current State Financial Management Circular. The Contractor also will negotiate value-add services including, but not limited to, parking and transportation. The Contractor will assist the State to source group reservations consisting of 10 or more hotel rooms. The Contractor may charge up to a $10.00 fee per room, with a maximum of $100.00 total for the group reservation. The Contractor shall provide international travelers with the locations of CDC health advisories and travel warning provided by the U.S. Department of State, safety information, excess baggage cost information, passport/visa requirements, and necessary forms and documents after booking and through departure.
