Doctor’s Notes. Request for sick leave with pay in excess of three (3) working days must be supported by a statement from an accredited physician. Management may require such a supporting statement for absences of three (3) days or less.

Doctor’s Notes. The Employer’s practice is not to arbitrarily ask for a doctor’s certificate when an employee is absent from work for less than three (3) days unless the employee’s record of employment indicates a pattern of absenteeism.

Doctor’s Notes. It is agreed that Doctors notes will be paid for by the Company in cases where these notes are requested by the Company and/or its Insurers. Employees must submit an invoice as proof of payment.
Doctor’s Notes. During current negotiations the Union raised concerns regarding supervision requesting employees to supply doctors statements in cases of continuing absenteeism. The Company stated that in cases of this nature, supervision are expected to use discretion and good judgement in making these requests of an employee. This method of controlling absenteeism should be used only when supervision has reason to believe that the employee's absence was without just cause.
